The FOPH has published the 2027 premiums: the average premium rises by +5.0% to CHF 412 a month. Here are all cantons, all insurers and your new premium by postcode.

  • The average premium rises by +5.0% to CHF 412 a month in 2027 (FOPH).
  • By age: adults +4.9%, young adults (19–25) +4.8%, children +4.6%.
  • The biggest increase is in the canton of Jura (+6.6%), the smallest in Glarus (+3.0%) (FOPH).
  • 308 of 1019 standard tariffs for adults rise by 5% or more, 25 go down.
  • In the city of Zurich, the cheapest and the most expensive insurer are up to CHF 2'587 a year apart, for identical basic cover.

    The FOPH has published the approved premiums of all 32 health insurers in its open dataset. The canton and insurer analysis and the calculator on this page are based on it.

    From 2027 the FOPH groups family-doctor and HMO models as «Praxis» (practice). New are «Flex» (several first points of contact to choose from) and «Pharmacy». Telmed and app models are listed as «Telmed/digital».

How much do health insurance premiums rise in 2027?

The average monthly premium for compulsory health insurance (OKP/LAMal, basic cover) rises by +5.0% to CHF 412 in 2027. That is CHF 19.70 more per month than in 2026, or about CHF 236 a year. The Federal Office of Public Health (FOPH) published the figures on 29 September 2026.

The FOPH calculates the average premium by adding up all premiums paid in Switzerland and dividing them by the number of insured people. Your own premium depends on where you live (canton and premium region), your age, your deductible, your insurer and your insurance model. That is why it rises much more for some people and less, or not at all, for others.

For a family with two adults and two children, that means about CHF 57 more per month on average (2 × 22.80 + 2 × 5.70), or about CHF 684 a year.

After +6.0% for 2025 and +4.4% for 2026, the average premium rises faster again in 2027. Comparis had forecast +3.7% in June, the FOPH itself +4.5 to 5%. At +5.0%, the result is at the top of the forecast range. The figures are shown as the FOPH published them each September.

Background

Why are Swiss health insurance premiums rising in 2027?

In short: because health costs keep rising. Premiums have to cover the expected costs of every insurer in every canton. Insurers calculated their 2027 premiums and submitted them to the FOPH, which checked them against the legal requirements and asked for changes where needed. The +5.0% increase matches the expected growth in costs.

Costs rose by 5.3% in 2025, again faster than the average since the Health Insurance Act (KVG) came into force. For 2026 the federal government projects +4.7%, and for 2027 +5.0%.

The cost drivers

  • More treatments: costs per patient are rising, mainly because more treatments are offered and used, even though the frequency of illness has stayed fairly stable.
  • Medical and technological progress: new treatments and therapies.
  • An ageing population: older people need more care on average.
  • Higher prices: since 2024, cantonal tariffs for outpatient and inpatient care have been rising as well.

A special factor: TARDOC

Since the switch to the new outpatient tariff system (TARDOC and outpatient flat rates), many bills arrive late. That makes the 2026 cost trend harder to estimate, but according to the FOPH there are no signs that the increase is slowing down.

What the federal government is doing about costs

By reviewing benefits, drug prices and tariffs, the Federal Department of Home Affairs (FDHA) and the FOPH say they have saved more than CHF 2 billion over the past ten years. Since 2024, Federal Councillor Elisabeth Baume-Schneider has brought the healthcare sector together at a round table, and the first measures are being implemented. At the end of 2026, the Federal Council will also set targets for cost and quality growth in basic insurance for the first time, for 2028 to 2031 (a result of the indirect counter-proposal to the cost-brake initiative). If costs grow faster than a target, the Federal Council and the cantons must consider corrective measures.

New in 2027

What else changes in 2027: new models, fewer insurers

New model categories: practice, Telmed/digital, Flex and pharmacy

With the 2027 premiums, the FOPH reorganises the insurance models with a restricted choice of doctor. It groups family-doctor and HMO models as «Praxis» (practice), because the two approaches had become almost identical. Flex and pharmacy models are new. The calculator on this page and priminfo.admin.ch use these categories.

Two fewer insurers: 2 mergers on 1 January 2027

From 1 January 2027, AMB Versicherungen AG and Caisse-maladie de la vallée d’Entremont no longer operate as separate health insurers: both merge with Mutuel Krankenversicherung AG (Groupe Mutuel). That leaves 32 insurers offering basic cover in 2027, down from 34 in 2026 (FOPH list of insurer changes 2027, German).

How to save on your premium in 2027

1. Switch health insurer

Basic cover is the same at every insurer, and every insurer has to accept you for basic insurance, so switching often pays off. Your cancellation has to reach your current insurer by 30 November 2026; the new insurer then covers you from 1 January 2027. Our switching guide walks you through it.

2. Check your deductible

If you rarely have health costs, a higher deductible (up to CHF 2,500) lowers your premium. If you have regular doctor or medication costs, a low deductible is often better. See how to choose your deductible.

3. Choose your insurance model

Practice, Telmed and Flex models are cheaper than the standard model because you first contact a designated point of care. According to the FOPH, you can change your model until the end of November. Our guide to the models explains the differences.

4. Apply for a premium subsidy

People on modest incomes are entitled to an individual premium reduction (IPV) from their canton of residence. Each canton sets its own rules and procedure.

5. Don't cancel supplementary insurance too early

Basic and supplementary insurance can be with different insurers. Only cancel supplementary cover once the new policy is confirmed in writing; supplementary insurance has its own notice periods in the policy conditions.

6. New to Switzerland?

Premiums have to cover the expected costs of every insurer in every canton, and those keep rising: by 5.3% in 2025, and the federal government expects +4.7% for 2026 and +5.0% for 2027. The main drivers named by the FOPH are more treatments per patient, medical and technological progress and an ageing population. Since 2024, higher cantonal tariffs for outpatient and inpatient care add to this. Costs for 2026 are harder to estimate because the switch to the new outpatient tariff (TARDOC and outpatient flat rates) has delayed many bills.

What do the new «Practice» and «Flex» models mean?

From 2027 the FOPH groups family-doctor and HMO models as «Praxis» (practice): your first point of contact is a designated practice or health centre. «Flex» is new and lets you choose between several first points of contact, for example a practice or telemedicine. Telmed and app models are now called «Telmed/digital», and there is a new «pharmacy» category.

When will I get my new premium for 2027?

Your insurer has to send you your new premium in writing by 31 October 2026 at the latest. The FOPH publishes all approved premiums at the end of September, so you can already compare every insurer in your premium region in the calculator.

What is the deadline to switch health insurer for 2027?

Your cancellation of basic insurance has to reach your current insurer by 30 November 2026 (send it by registered post, early). You are then insured with the new insurer from 1 January 2027. Supplementary insurance has its own notice periods, set out in your policy conditions.

Often, yes: basic cover is identical at every insurer, the premiums are not. In the city of Zurich the cheapest and the most expensive insurer are up to CHF 2'587 a year apart in 2027. The right deductible and a managed-care model (practice, Telmed, Flex) often save several hundred francs more. If you have supplementary insurance, have it checked before you switch.
